I would highly recommend the books by Sherry Rogers:

The Cure is in the Kitchen: the strict healing phase of the macrobiotic diet

and

Macro Mellow

She has others, but I believe you will find everything you need in the first book I have listed. You are What you Ate is also wonderful, full of case histories if I am remembering correctly (I don't own that one yet but read it on inter library loan).
